Health Clinic - "Patient Hand Held Records"
A grant of $3,980 was sought to purchase 1000 Patient Held Records for refugees that attend the clinic. The health record, developed by the Southern Division of General Practice, gives an in-depth analysis of a client's health needs and makes it clear who is working with the client, what health needs have been identified and the future care plan. This makes it much easier for the client when, after initial assessment and treatment at the QIRCH clinic, they are referred for specialist hospital appointments or to a community GP for ongoing care and management. Each subsequent health care worker or doctor has the client's medical
history readily available and is aware of previous illness, conditions
and treatment. The record helps to reduce the service provider & interpreter
time spent on repetitious gathering and dissemination of information.
It also protects the patient from having to relive their trauma each time
